PURPOSE AND VISION

We help everyone in our community receive better care every day, in every way.

North Western Melbourne Primary Health Network

We are part of a nation-wide group of Primary Health Networks funded by the Australian Government. Our role is to improve the health of people who live in our region.

Everything we do is aimed at improving the health and wellbeing of our community. If people need support or services that don’t exist, we fund health services to fill the gap.

Aboriginal people have had a deep connection to this region for many thousands of years. We acknowledge the people of the Kulin nations as the Traditional Custodians of the land on which our work in the community takes place. We pay our respects to their Elders past and present.

We plan and fund several programs to support Aboriginal and Torres Strait Islander people to better manage complex and chronic health needs, including those related to mental health and alcohol and drug use.

We also work in partnership with Aboriginal and Community Controlled Health Organisations and general practices to improve health outcomes and access to culturally sensitive health care.

You can learn more about our Aboriginal health programs and resources on our website. You can also find more information about Aboriginal health in our region on our Community and Population Health Profiles page.
