This position engages and develops relationships with Aboriginal and Torres Strait Islander people with complex physical health and social support needs who face significant barriers to accessing services. This includes engagement, intake, assessment, interim response and service linkages, case management, care planning with tailored psychosocial and health supports to individuals and discharge planning both through site based and assertive outreach activities.
